Please feel free to submit a paper form OR a digital form - please only submit one or the other, NOT BOTH!

Please also submit one form PER EVENT, even if the items and prices are the same.

Lastly, please skim over ALL TERMS prior to each event, as there may be small additions/edits in between events. If there are any MAJOR edits that you should be aware of, we will notify you as we do not want any surprises!
